Nurse (Case Management)
Applies case management essential activities assessment, planning, implementation, coordination, monitoring, evaluation, outcomes across the continuum of care. Maintains current knowledge of resources available within the community, maintain supply of resource material to be distributed to patients when needed. Provide technical assistant to PRC Staff for clinical issues for their assigned patients. Makes rounds of patient care areas to review charts for appropriateness, completeness, and timeline of documentation by providers. Reviews medical records from outside referrals for recommendations and follow up care. Qualifications:

Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:
Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care. OR; successfully completed a PH.D or equivalent doctoral degree from a professional nursing educational program or related medical science field. You must meet all qualification requirements within 30 days of the closing date of the announcement.
Estimated Salary: $20 to $28 per hour based on qualifications.
